T-CARE

Are you feeling stressed or overwhelmed taking care of a loved one?  We are here to help!  The Social Services team is ready to provide personalized solutions to support you in providing the best care possible for your loved one. 

Please complete the assessment form by clicking HERE and a member of our team will follow up.  If you prefer, call (630) 483-5660 and a member of our team can assist you.  The assessment identifies needs and a personalized care plan is created.  The T-CARE assessment was created by the Administration for Community Living to prevent burnout of caregivers and reduce stress.

Caregiver Support Group

  • Monthly support group meets 4th Wednesday of every month at 10:30 AM.
  • Caregivers of a spouse, a friend, or a loved one are welcomed.
  • Meetings are either educational or open for discussion.
  • This group is facilitated by Social Services Specialist Melissa Endean
  • There is no fee to attend.

FREE On-Line Training

  • Trualta provides on-demand training for caregivers at no cost.
  • Videos are available 24/7.
