The Hanover Township Home Care Program is intended to assist older adults who need additional services and support to remain in their homes and prevent unnecessary nursing facility placement.

Program services include:

• Assistance with activities of daily living (bathing, dressing, grooming, etc.)
• Light housekeeping
• Grocery shopping and meal preparation
• Companionship including engaging with programs at the Senior Center
   or connecting to virtual programs
• Support using transportation services

A Township nurse and the Home Care Coordinator will provide an initial
in-home evaluation for participants to develop a service plan based on a
resident’s needs.

Services are provided on a sliding scale based on a resident’s asset level.
Hourly costs range from $5 - $25 per hour. 

This program is licensed through the Illinois Department of Public Health.
