Five Wishes

Do your family and friends know what your wishes are for end of life care? Staff from Seasons Hospice and Palliative Care will present this informational session. The program will discuss the need to make your wishes known, how to choose the best person to execute your wishes, and will help you plan what those wishes might be. You will leave with a legal document stating what your advanced care planning options are. Registration required.
